# Dual-Eligible Integrated Care SEP: Monthly Enrollment for 12.8 Million Beneficiaries

Effective January 2025, full-benefit dual-eligible beneficiaries can enroll in integrated D-SNPs — FIDE SNPs, HIDE SNPs, and AIPs — once per month via the Integrated Care SEP. This replaced the former quarterly Dual/LIS SEP for MA enrollment and opened a true 12-month enrollment window for approximately 12.8 million beneficiaries nationwide.

## What Changed and Why It Matters

The old Dual/LIS SEP allowed quarterly MA plan switching. The revised structure splits the pathways: the Integrated Care SEP handles monthly D-SNP enrollment, while the Dual/LIS SEP now only permits monthly PDP switching without MA enrollment.

For agents, this is a fundamental shift. Dual-eligible beneficiaries are no longer limited to quarterly windows. Every month is an enrollment opportunity, creating 12 chances per year to place each eligible beneficiary.

## The Dual-Eligible Population by the Numbers

The aged dual-eligible population — those 65 and older with both Medicare and Medicaid — totals approximately 4,791,100 individuals. This population is concentrated in states with large Medicaid programs and high poverty rates among seniors.

Key market characteristics for dual-eligible campaigns include:
- **D-SNP Gap:** Counties where D-SNP supply does not match demand
- **LIS Opportunity:** Areas with high Low-Income Subsidy eligibility
- **SDOH Burden:** Markets with elevated social determinants of health challenges
- **Medicaid Loss:** Counties experiencing ongoing Medicaid eligibility redeterminations

## Building a Monthly D-SNP Pipeline

The monthly enrollment window means agents need a different approach than AEP-style sprints. The most effective strategy is a steady-state pipeline that produces consistent monthly enrollments:

**Lead Generation:** Partner with community organizations, food banks, and social services agencies that serve the dual-eligible population. These organizations see eligible beneficiaries regularly and can facilitate warm introductions.

**Education First:** Dual-eligible beneficiaries often have complex healthcare needs and may not understand their enrollment options. Leading with education — explaining what a D-SNP is, what integrated care means, and how it differs from their current coverage — builds trust and converts at higher rates.

**Monthly Touchpoints:** Since the enrollment window resets monthly, maintain regular contact with prospects who are not yet ready to enroll. A beneficiary who says no in March may say yes in June when their circumstances change.

## How unLocked CRM Manages Dual-Eligible Outreach

unLocked CRM segments dual-eligible prospects by their specific integrated care eligibility (FIDE SNP, HIDE SNP, AIP) and tracks monthly enrollment windows automatically. The communication engine schedules recurring touchpoints aligned with each monthly enrollment cycle, ensuring no eligible beneficiary falls through the cracks.
